    Prices are discussed before evaluation or treatment. When treatment options are discussed, the cost of each will be as well.

  • Fee for service

    Payments will be made prior to services rendered for evaluations and treatments. Which means no hidden fees or bills after each visit.

  • No insurance

    All payments are made prior to or day of service. No insurance claims will be made on a patient’s behalf. This allows for more time for evaluation and treatment and freedom in developing individualized treatment plans.

EVALUATION PRICES

COMPLEX Initial (120min): $500
Includes limited diagnostic ultrasound if indicated and review of imaging and medical records up to 20pages*, and discussion/coordination of care with other involved providers after the visit.
For persons who have:

  • seen 2 or more providers/physicians

    • multiple imaging studies

    • pain in more than 1 body part or region

    • had interventions/procedures

  • STANDARD Initial (60min) $300
    Includes limited diagnostic ultrasound if indicated.
    For persons who have:

    • Less than 1 providers seen prior

    • 2 or less body parts/regions involved

    • No imaging or 1 XRay

  • FOLLOW-UP Appointments (30min, 60min, 90min) $100 per 30min

  • Independent Medical Exam, please contact for pricing

  • Sports Physicals, up to 30 minutes- $75
    — Purpose of this visit will be to complete the necessary Pre-Participation Physical Exam (PPE) paperwork. We may discuss eligibility and recommendations on finding for sports participation. This is evaluation only and no treatment of abnormal findings is included.

NOTES:
*additional $5 for every additional 0-10pages records to review
—Injections/procedures same day as Office Visit are a separate charge. Cannot guarantee injection same day as office visit. Any procedure not performed on day of office visit will not require additional office visit charge for procedure only.

—Due to complexity of some problems and extensive medical records, multiple visits may be required in order to obtain a full history and complete exam on each area of concern.

PROCEDURE PRICES

  • Cost of procedures are discussed after thorough evaluation and discussion of options.

  • Prices include ultrasound guidance, supplies, medications, procedural time, facility fees, and post-procedural instructions.

  • Costs of procedures (not incl. PRP) range from $125 to $350. Many factors go into cost including complexity/risk of injection, time needed for injection, facility fees, and cost of supplies.

  • PRP is highly individualized based on diagnosis, area to be injected, individual co-morbidities, and volume needed to adequately treat specific area(s). Prices range from $650-$1,400. PRP must be scheduled ahead of time and cannot be added on the same day as an evaluation. Please see PRP page for more information.

LATE CANCELLATION
NO SHOW POLICY

We understand that situations arise in which you must cancel your appointment. If you need to cancel or re-schedule your appointment, please do so 24 hours prior to your scheduled visit.

LATE CANCELLATION is defined as cancellations occurring less than 24 hours before the scheduled visit. Late cancellations may be subject to a $50 fee for office visits and procedures. 

NO-SHOW is defined as notification of cancellation within 1 hour of the appointment time or not showing up to appointment within 15 minutes of appointment start time.  No-Shows are subject to a $100 fee for office visits or $100 plus the cost of the supplies of the scheduled procedure.

Any of the above fees will be required to be paid prior to re-scheduling a visit. 

Patients who NO-SHOW or LATE CANCEL will be required to pay their estimated visit amount at the time of scheduling and prior to any procedures being performed.

Patients who NO-SHOW or LATE CANCEL two (2) or more times in a 12 month period may not be offered any further appointments at Equity Orthopedics and Sports Medicine.

OBTAINING RECORDS

We cannot obtain records from other facilities for you. We will provide guidance on how to obtain records on your own and how to have them sent to us. It is highly recommended that you have printed copies of your medical records and store them in an labeled accordion folder in a three-hole punched binder.

Outgoing records will be sent free of charge in digital formats. Paper copy of records will be printed and sent for $0.25 a page plus cost of postal fees.
